What is human in the loop?

The practical middle between full automation and doing everything by hand. The design question is not whether to include a person but exactly where: reviewing every output, approving before anything leaves the building, or taking over only on exceptions. The wrong placement is worse than either extreme, because somebody asked to rubber-stamp a hundred items a day stops reading them, which produces the risk of automation with the cost of manual work.
